WISe Services

The WISe Program targets youth who are at risk of being increasingly involved with other systems because of challenging behavioral health needs. Our team works collaboratively with families and community partners to develop creative, individualized plans that build on family strengths and values that allow these youth to remain safely in their homes and connected to and engaged in their community.

The focus of WISe is to provide intensive mental health services to support youth and help families reach their shared vision for a healthy and happy life. WISe is home and community based, and services and supports are offered in locations and times that work best for the youth and their family. Once enrolled in WISe, families and youth have access to crisis services any time of the day, 365 days a year.

Eligibility Criteria

  • Youth must have a mental health diagnosis.
  • Youth must be covered by Medicaid.
  • Youth must be under the age of 21.
  • Family and youth are voluntarily willing to participate.
  • Past and current services have been unsuccessful, and youth and family are in need of a creative, high-intensity program.

  • Displaying an elevated risk of harm to themselves or others.
  • In need of more intensive and individualized approach to treatment.
  • Involved in multiple systems (i.e. CPS, DDA, JJC, Special Education).

All referrals include:

  • Completing the Child Adolescent Needs and Strengths (CANS) Screen.
